Request immunisation for students and staff

Requests for immunisation or disease screening evidence must be submitted through my.UQ for any student or staff member with a mandatory requirement.

For students

UQ staff are responsible for initiating student immunisation requests via my.UQ. This should be assigned to dedicated staff, as selecting the correct category and immunisation requirements is critical. Placement or academic administration teams can advise on faculty-specific processes.

If you’re unsure about the process, visit the Systems Training Hub (staff login required) or contact the safety network staff or Immunisation Records Team before making a request.

  1. Initiate a request in my.UQ
    Initiate a Student Immunisation Evidence Request in my.UQ for coursework students. You can submit a request for one or multiple students in a single form. If all students share the same due date and immunisation requirements, create a cohort request that will generate an individual case for each student.
  2. Nominate a key contact
    When submitting a request, nominate a key contact (faculty, school or staff member). For cohort requests, assign the key contact as an organisational unit, allowing multiple staff in that unit to monitor and update cases via UniTask in my.UQ.
  3. Monitor student compliance
    Organisational units should use UQ Reportal to track student compliance. The key contact is responsible for following up with non-compliant students before potential exposure to a vaccine-preventable disease.

For HDR and Honours by research students

If you’re a principal advisor or supervisor, you’re responsible for initiating an immunisation request for your research students.

If you’re unsure about the process, contact the safety network staff or Immunisation Records Team before submitting a request.

  1. Initiate the request in my.UQ
    Initiate a Student Immunisation Evidence Request in my.UQ for individual research students. Safety network staff can also submit requests on your behalf.
  2. Nominate a key contact
    Nominate a key contact (usually the principal advisor, supervisor or delegate) to monitor compliance. Once evidence is submitted, the Immunisation Records Team (IRT) will assess and verify the documents. If further information is required, the student will receive automatic notifications via email or my.UQ.
  3. Monitor compliance and follow up
    Both the request initiator and key contact can monitor the case in my.UQ and UQ Reportal. The key contact must follow up with any non-compliant students before potential exposure to a vaccine-preventable disease.

For staff

If you’re a supervisor, you’re responsible for initiating immunisation requests for your staff members.

If you’re unsure about the process, contact the safety network staff or Immunisation Records Team (immunisation@uq.edu.au) before making a request.

  1. Initiate the request in my.UQ
    Initiate a 'Staff Immunisation Evidence Process' in my.UQ for individual staff members. Safety network staff can also initiate requests on your behalf.
  2. Nominate a key contact
    Nominate a key contact (yourself or a delegate) to monitor compliance. After submission, the Immunisation Records Team (IRT) will assess and verify the documents. If any evidence is missing, the staff member will receive automatic notifications via email or my.UQ.
  3. Monitor compliance and follow up
    The initiator and key contact can track progress through my.UQ and UQ Reportal. The key contact must follow up with any non-compliant staff before potential exposure to a vaccine-preventable disease.
